Main duties and responsibilities
Clinical assessment and management
- Assess and manage the full breadth of clinical presentations and age groups in an urgent primary care setting, including physical and mental health needs.
- Undertake clinical consultations by telephone and, where appropriate, video, consulting with patients and/or their representatives and community healthcare professionals to assess concerns, establish a differential diagnosis, and agree a management plan.
- Support community-based management wherever possible, while recognising serious illness and emergency situations that require admission and/or an ambulance response.
- Prescribe appropriately in an urgent primary care context, including initiating and adjusting end of life medications.
Referrals and system navigation
- Take telephone referrals from healthcare professionals (HCPs) across the Bristol, North Somerset and South Gloucestershire (BNSSG) area for appropriate adult medical admissions, while remaining mindful of capacity pressures within the hosting acute trust.
- Build and maintain positive working relationships with referring HCPs, using excellent communication skills to explore alternative management options constructively and without confrontation.
- Develop and maintain familiarity with other urgent care services available across BNSSG, and how to access these for patients.
Documentation and clinical governance
- Document key findings from assessments clearly and contemporaneously, ensuring records provide sufficient detail and support other clinicians involved later in the patient’s journey.
- Be proactive in identifying and reporting concerns and incidents, supporting learning and continuous improvement in the service provided to patients.
Team working and support
- Seek clinical advice from the senior GP on shift (the Clinical Coordinator) as needed to support decision-making and access to appropriate services.
- Buddy with new clinicians working their first shift in the Severnside service, when required, helping them become confident with the systems and processes following induction.
- Contribute to on-call clinician rotas during periods of peak demand, where possible.
Digital systems and tools
- Use all IT systems required for safe clinical work in Integrated Urgent Care (IUC), including clinical systems (Cleo, PACCS and EMIS) and decision-support resources (Clinical Toolkit, BrisDoc weblinks, BNSSG Remedy).

Qualifications and training
- Full registration with the General Medical Council (GMC) and on the GP Register.
- Inclusion on NHS England’s Performers List.
- MRCGP or a Certificate from the Joint Committee on Postgraduate Training for General Practice.
Knowledge and experience
- Experience of, and interest in, working across urgent primary care settings.
- Currently working in General Practice or another form of community medicine or an Integrated Urgent Care (IUC) service.
- Evidence of effective multidisciplinary team working as part of current or recent primary care experience, either in practice or IUC, or both.

Desirable criteria
Qualifications and training
- Higher postgraduate membership/qualification (for example, MRCP or MSc).
Knowledge and experience
- Experience of teaching medical students.
- Experience of GP training and/or clinical supervision.
- Familiarity with the BNSSG healthcare system.
- Acute and/or general medical experience.
- Experience of referral management and primary care/hospital interface decision-making.
Skills, abilities and attributes
- Familiarity with Cleo and EMIS systems.
