About the Role

The Lead Clinical Practitioner (LCP) role for The Burren is a leadership role within Severnside Integrated Urgent Care (IUC) where part of the job role will entail advanced clinical practitioner functions within The Burren as well as IUC Out of Hours, System Clinical Assessment System (CAS), Clinical Medication Delivery Unit (CMDU) and Frailty- Assessment and coordination for emergency and urgent care (ACE).

The proposed clinical hours within The Burren will be made up of 10.5 hours per shift (Monday to Friday) and a four weekly 4-hour shift on Saturday which equates to 17.75 hours per week on average. There will also be 6 hours of dedicated leadership time per week. Further Severnside IUC hours can be negotiated and will be variable.

Part of your role will be within The Burren.  The Burren facilitates an exciting new collaborative service between secondary, community, urgent and primary care in the intermediate care space providing an innovative patient journey.  This role requires the post holder to be an autonomous prescribing clinician providing primary medical care whilst working effectively within their professional boundaries.  The post holder will be responsible for managing the day-to-day clinical needs of a case load of patients being discharged from secondary care for community placement with rehabilitation needs.  The role will require the clinical practitioner to have advanced practice skills and training to enable clinical risk holding and a high degree of autonomous decision making.  It will be advantageous to have chronic disease management experience as well as urgent care experience.  The clinician will be supported by the Nursing & AHP leadership team, primary care GPs and will provide exemplary care for their patients, priding themselves on meeting individual patient health needs.

As a LCP, you will be managing a team within Severnside IUC and supporting Clinical Practitioner colleagues with supervision, mentoring and clinical support.  The LCP will also lead on aspects of clinical governance, clinical audit, service improvement, clinical care and patient safety as well as contribute hours to the clinical rota.

The LCP will act as a key member of the multi-disciplinary team (MDT) in The Burren, participate in ward rounds and focus on the management of routine and urgent patient needs.  The Lead Clinical Practitioner will work collaboratively with the wider clinical and operational team (Nurses, AHPs, GPs, Therapy, Social Care and Operational Colleagues) to support individual patients and the delivery of the right care first time.
