Why this role matters
- You’ll support clinicians to make safe, confident decisions in real time, including holding risk, managing uncertainty and responding to safeguarding concerns.
- You’ll support admission alternatives through the Weekday Professional Line (mainly adult medical admission avoidance) and by promoting the right community pathways.
- You’ll help keep more patients cared for in the community where it’s safe to do so, including supporting referrals to Frailty-ACE and NHS@Home (“virtual wards”).
A Diverse clinical mix
Working as a Clinical Coordinator (CC) is varied and interesting, with lots of interaction with clinical and operational colleagues in IUC and System CAS, plus consulting with patients and healthcare professionals.
You’ll offer accessible clinical advice and support across elements of Severnside IUC, including:
- Mental Health Integrated Access Partnership (IAP): supporting mental health colleagues with physical health assessments and prescribing
- System Clinical Assessment Service (CAS): a multidisciplinary team supporting remote clinical assessment of patients who might otherwise be directed to emergency department or ambulance dispositions
- Weekday Professional Line: mainly adult medical admission avoidance, plus some patient consultations
- Covid Medicine Delivery Unit (CMDU) the remote assessment of patients referred for consideration of covid antiviral medications.
- Frailty-ACE (Assessment & Coordination for Emergency and urgent care): assessing frail patients who might otherwise be conveyed to hospital and coordinating community pathways where appropriate
- Out of Hours (OOH): telephone/remote consulting, face-to-face appointments and home visits
Portfolio option: work solely as a Clinical Coordinator, or undertake Clinical Coordinator shifts as part of a wider portfolio in Severnside or BrisDoc’s practice services.
What you’ll do
You’ll be a strong team player who builds collaborative, supportive working relationships across clinical and operational colleagues, helping our teams deliver excellent care for patients.
Workstream 1: Senior clinical advice and decision support
- Provide accessible clinical advice and support to clinicians across Severnside IUC teams.
- Support decision-making on prescribing, complexity, uncertainty, risk holding, live safeguarding concerns, and appropriate referral pathways.
- Discuss options, share decision-making, and negotiate with healthcare professional colleagues to meet patients’ needs.
Workstream 2: Keep urgent care flowing safely
- Work in close liaison with the operational Shift Manager as part of joined-up clinical and operational leadership on shift.
- Provide advice and support to the Shift Manager where needed, alongside operational and clinical support structures.
- Review cases put through for appointments or home visits by triaging clinicians in OOH prior to booking.
Workstream 3: Targeted clinical assessment and pathway leadership
- Assess and manage patients referred into the System CAS service.
- Assess and manage patients referred into the CMDU service.
- Provide clinical cover to the Weekday Professional Line and Frailty-ACE teams if there is a staffing or workload challenge.
- Take a proactive approach to supporting urgent care needs in the community, including promoting referrals to Frailty-ACE and NHS@Home (“virtual wards”).
Some shifts are fast-paced. Your role is designed around strong clinical and operational collaboration, so you’re not holding the system alone.
Support and development
- You’ll be encouraged to supervise GP trainees, with training available to become an approved Clinical Supervisor.
- Engage with and support service change and improvement, supported by the leadership team.
The systems you’ll use
You’ll use the IT systems required for safe clinical work in IUC, including clinical systems Cleo, PACCS and EMIS, plus decision-support resources such as the Clinical Toolkit, BrisDoc weblinks and BNSSG Remedy.

What you’ll need
All GPs working in IUC are expected to meet the criteria set out in our Severnside IUC GP main duties and person specification. Over and above this, for the Clinical Coordinator GP role we are looking for:
Essential (in addition to the Severnside GP role criteria)
- Urgent primary care experience and strong consultation skills, including the ability to navigate complexity, use person-centred and shared decision-making, and hold risk and uncertainty.
- A proactive approach to supporting urgent care in the community and promoting admission and hospital avoidance where safe.
- Strong communication skills to ensure discussions with clinical colleagues about assessment and management are supportive, focused on patient care, and include the ability to negotiate and hold risk appropriately.
- Ability to manage multiple parallel workstreams and prioritise appropriately.
Desirable (in addition to the Severnside GP role criteria)
- Experience in training, clinical supervision or coaching.
- Previous experience working as a GP in Severnside IUC services, for example Out of Hours (OOH), Frailty-ACE (F-ACE), Weekday Professional Line (WDPL) and/or System CAS.
- Awareness of the urgent care landscape in BNSSG (Bristol, North Somerset and South Gloucestershire).
