We are pleased to announce, the Frailty ACE Programme, has restarted for the next four months starting from the 10th of January.
The core objective of Frailty ACE is to address urgent care needs directly within the patient’s home environment, wherever feasible and in keeping with the patient’s preferences and best interests. By harnessing the collective expertise at Osprey Court and effectively coordinating the appropriate services, we aim to foster a comprehensive, patient-centric approach. This initiative emphasises collaborative efforts and transcends conventional organisational and service boundaries, enabling us to manage uncertainties, mitigate risks, and minimise potential harm to our patients.
The Team and Operations
The team comprises three SevernSide IUC Primary Care clinicians, a Sirona Advanced Clinical Practitioner (ACP), and a Social Worker. Located in the Pier Room, the team operates closely with the System CAS, Mental Health, Weekday Professional Line, and Operational Teams. The services operational hours are from Monday to Friday, 9 am to 6 pm.
