Join our Weekday Professional Line as a GP providing senior clinical advice to support safe care in the community.
This is a senior, weekday GP role at the primary–secondary care interface. You’ll speak with GPs and other community clinicians about acute medical presentations, agree a clear plan, and support the best next step for each patient. You’ll help teams avoid admissions where safe, and when hospital care is needed, support timely referral via the right route.
Based at Osprey Court, you’ll work within a supportive multidisciplinary team alongside System CAS colleagues. The role is varied and offers a strong mix of clinical thinking, professional collaboration and system knowledge across BNSSG.

About the Weekday Professional Line
The WDPL team operates 08:00–18:30, Monday to Friday, and is based at Osprey Court. We receive calls from primary care clinicians and other community healthcare professionals seeking advice or admission for adult patients with acute medical presentations.
Our key function is to:
- explore management pathways not yet considered that could support safe community management, and
- facilitate admission to hospital via the most appropriate route when required.
We provide this service for patients within the catchment of the three BNSSG acute hospitals: Southmead Hospital, Bristol Royal Infirmary and Weston General Hospital.
You’ll also receive a small number of cases from NHS 111 during the day (including poisonings), and you’ll work alongside the System Clinical Assessment Service (System CAS) team, supporting remote clinical assessment for patients who might otherwise be directed to ED or 999 without clinical input.
What you’ll do
Workstream 1: Senior decision support for community clinicians
- Receive telephone referrals for medical admission, carefully exploring the presentation and the referrer’s concerns.
- Use shared clinical decision-making and critical thinking to agree a clear, patient-centred plan.
- Provide medical advice and support to community clinicians managing adult medical patients in the community.
- Signpost to specialist input and community management pathways when helpful.
Workstream 2: Admission avoidance, frailty and complex presentations
- Aim to support community management wherever safe, particularly for patients with terminal illness and/or advanced frailty.
- Facilitate referral to Frailty-ACE for frail patients who would benefit from further discussion on community management options.
- Seek specialist advice where needed to support admission avoidance.
Workstream 3: Right-route hospital access when admission is needed
- Facilitate urgent/same-day hospital referral via the most appropriate hospital pathway.
- Maintain clear, supportive communication with referring clinicians and colleagues in affiliated organisations.
Workstream 4: Varied remote clinical assessment (System CAS and selective NHS 111)
- Remotely assess and manage a small number of NHS 111 cases, including accidental toxic ingestions and people aged under 2 or over 85 with unclear needs.
- Work alongside System CAS colleagues to assess patients who might otherwise be directed to a Category 3/4 ambulance or Emergency Department without clinical input.
Service improvement, team work and teaching
- Take a proactive approach to case discussion and supporting clinical decision-making across WDPL, Frailty-ACE and System CAS.
- Contribute to the development of new pathways and service improvements that enhance patient care.
- Support the assessment and management of Covid Medicine Delivery Unit (CMDU) patients if required.
- Opportunity to support supervision for GP trainees shadowing or training in WDPL (Clinical Supervisor training can be provided if required).

What you’ll need
All GPs working in IUC are expected to meet the criteria set out in our Severnside IUC GP main duties and person specification. Over and above this, for this Weekday Professional Line GP role we are looking for:
Essential (in addition to the Severnside GP role criteria)

Confident advising healthcare colleagues on the management of common acute medical presentations.

Strong communication skills to support shared decision-making with healthcare professional callers about patient care and pathways.

Able to recognise when specialist advice is required, and seek it promptly.

Able to interpret common investigations, including blood results and ECGs.
-
Able to attend occasional evening team meetings or staff training sessions.
-
A clear interest in acute and urgent care.
Desirable (in addition to the Severnside GP role criteria)

Experience in urgent or emergency care (for example Emergency Department, Acute Medicine, Integrated Urgent Care or Out of Hours).

Interest in frailty and supporting safe community management for complex patients.

Knowledge of BNSSG acute care pathways.
-
Experience of referral management and/or working at the primary–secondary care interface.
